Police Health is proud to partner with Vitalis Health, a leading provider of fully integrated nurse-led clinical and care services, to offer this bespoke TeleNurse Surgery Support Program to eligible members at no additional cost. 

Staffed by highly trained and empathetic nurses with specific experience in the medical procedures covered (see table below), our TeleNurse Surgery Support Program offers pre-hospitalisation and post-discharge home support via phone. 

Program inclusions

Through a pre-admission and post-admission phone call, our TeleNurse team provide tailored support covering: 

  • Self-care
  • Navigating the healthcare environment
  • Understanding warning signs
  • Understanding post-discharge instructions
  • Compliance with medicine
  • Ensuring there are supports at home
  • Access to supportive programs
  • Liaising with GP or Treating Specialist, if required and with consent. 

This specialised support is designed to help each patient confidently manage the transition from home to hospital, and from hospital to home, improving health and psychosocial outcomes, as well as avoiding unnecessary hospital re-admissions.

  • To enter the TeleNurse Surgery Support Program, Members must be claiming eligible Police Health Hospital benefits for a Cardiac, Oncology, Orthopaedic, or Urology Procedure listed in the table below.

If you've agreed to being contacted by us and provided a contact number when you joined as a member you automatically qualify for the program, and you don't need to do anything more! Our TeleNurse Surgery Support team will give you a call prior to your hospital admission once we receive confirmation from your chosen hospital that you have a planned admission which you will be claiming eligible benefits for.   

No, you can opt-out at the initial phone call.  However, 98% of people on similar programs provided by Vitalis Health have taken this up and found it to be invaluable. 

If you’re currently in the TeleNurse Surgery Support Program, you’ll be given a dedicated phone number to call through to if you need support outside of the planned call times. For emergency care, please call 000.

No, this program is being provided at no additional cost to eligible members. This is because the program has been developed to pay for itself.   The goal is to improve your health outcomes and reduce numbers of avoidable hospital re-admissions and length of hospital stays.
