If you're a couple or family with a combined annual income greater than $140,000 or single with an annual income greater than $70,000, and don't have private hospital cover, you'll be charged a Medicare Levy Surcharge of 1% on your income (in addition to the normal 1.5% levy).

Please be aware that both partners in couples or families need to have hospital cover, or you'll both be subject to the levy.
